I would be glad to provide you a superbill, which includes my tax identification number, LCSW #, all other pertinent information and a diagnosis that we will discuss beforehand.
You may submit this bill to your insurance and seek your own reimbursement.
Reduced fee services are available on a limited basis.
Payment: Cash, check, Zelle, ApplePay and credit cards accepted for payment. Credit cards will be assessed $175.00, which includes a processing fee.
Cancellation/No Show Policy: If you do not show up for your scheduled therapy appointment, and you have not cancelled at least 24 hours in advance, you will be required to pay the full cost of the session. Please understand that your appointment time is reserved specifically for you, and unlike a doctor's office, no one else is scheduled during that time.
